Hi the tool was never intended for single monitor...that is why the Eyefish effect..try lower your FOV alot:) For the newest plugin update, dopefish is re-writing the plugin for the latest version of his tool;)) I believe when it is done he will post it here;)) or on WSF site;))

I'm using the plugin and it works, using
origin and a single display 1920x1080 but there's a few step to make it work,
1. after activating the plugin in-game, i have to zoom in then zoom out and the result is fisheye effect also the character in the menu is smaller
2. then i hit the activating button again, the the result is much more acceptable which
only when i'm using a very strange setting in the widecreen fixer plugin
the setting are : 12 for FOV and a 9600 by 1080
( the value in FOV doesn't seem have an effect for me, so i try changing the hoz value which is 9600=1920*5)
the reason is when i'm using 60-90 for FOV and 1920x1080 the result were ok in-game but very zoom-out view in cut-scene and a very small character avatar in menu.
