Does this also happen when you take the receiver out of the chain and hook the monitor up directly? I've had issues with receivers hooked to PC's via HDMI in the past. It's usually been due to problems with the EDID during the HDMI handshake. If this is the case you can try to setup an EDID override...

As promised, here is the HDMI splitter that I use. It comes in 3 flavors: 1 input into 2, 4, or 8 outputs. I personally use the 4 port splitter but any of them should work. There are possibly (probably) cheaper ones out there, but this is the only one where I can personally vouch for it's HDCP strip...

It doesn't matter which ports you have on your Titan. All he is doing is just running the AverMedia RECentral app in full screen mode, which is spanning the preview window across all three monitors, just like you can do with any other window when using Surround. Tispe my understanding is that Nvidia doesn't currently support accessory displays when using 3D Surround. 2D Surround only.

What you are describing is a normal phenomenon called fisheye, and is a side effect of the way the majority of games scale FOV. I have used Display Fusion to split my 5760x1080 into three virtual displays so that windows maximize to their respective virtual monitors. But it goes bananas with the taskbar, kinda buggy and it is not free. Nvidia has a software balled nView but it is disabled for GTX cards. I force installed it...
